Appointment Cancellations
If you cannot keep a scheduled appointment, please notify us at least 24 hours in advance. If you must cancel your appointment due to last-minute unforeseen circumstances, please let us know immediately.
What to bring to your appointment
New patients are always welcome. Our board certified physicians will perform a medical evaluation, review your medical records/test results and discuss your treatment options. We are also happy to coordinate your Houston cardiac care with your primary and/or referring physician.
We encourage you to contact our Scheduling Department at 281.956.7070 and arrange your appointment as far in advance as possible. Our Scheduling Coordinator will discuss your appointment time options. You will need your insurance card available when you call us. Our Clear Lake physicians may recommend testing depending on your symptoms or diagnoses. Cardiac testing can often be performed on the same day as your appointment or a separate day – depending on your preference.
Please arrive at least 15 minutes early for your appointment, and bring along the following items
- Health insurance card(s)
- Referral form (if required by your insurance company)
- A list of your current medications, including any vitamins and/or herbs -
(be sure to include the dosage and how often you take it) - A list of your medication allergies
- Completed patient registration form detailing your medical history
- Any required payment
It is your responsibility to fax your medical records to us at 281.956.7098 prior to your appointment. We need pertinent records relating to your cardiac history, including any cineangiogram or x-ray films.
